Semaglutide was first approved as a diabetes treatment medication and more recently gained an indication for weight loss based on evidence from several studies in both diabetic and non-diabetic patients. There are multiple mechanisms of action, specifically weight loss is achieved due to slowing gastric emptying time. By slowing digestion, the stomach sends signals of “fullness” to the brain which regulates appetite. Decreased caloric intake further leads to improved glycemic control increasing patient perceived energy and quality of life.
DOSING:
CONSERVATIVE Dose Escalation Schedule:
Month #1: Inject 0.1 mL (Semaglutide 0.25mg /L-Carnitine 10 mg) subcutaneously once a week Month #2: Inject 0.2 mL (Semaglutide 0.5mg /L-Carnitine 20 mg) subcutaneously once a week Month #3: Inject 0.4 mL (Semaglutide 1mg /L-Carnitine 40 mg) subcutaneously once a week Month #4: Inject 0.7 mL (Semaglutide 1.75mg /L-Carnitine 70 mg) subcutaneously once a week
Maintenance Dose: Inject 1 mL (Semaglutide 2.5mg /L-Carnitine 100 mg) subcutaneously once a week
Mechanism of Action:
Semaglutide is a long-acting GLP-1 receptor agonist that increases glucose-dependent insulin secretion, decreases inappropriate glucagon secretion, slows gastric emptying; also acts in the areas of the brain involved in regulation of appetite and caloric intake.
Adverse Effects:
As with other GLP-1 receptor agonists, adverse effects GI effects are common (~5%); the major adverse effects are gastrointestinal, including nausea, diarrhea, and vomiting. In weight loss trials, these adverse effects were generally mild to moderate and, for most patients, improved over time.
Contraindications and Precautions:
Semaglutide is contraindicated during pregnancy and in patients with a personal history of pancreatitis or a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia 2A or 2B. In addition, for patients taking semaglutide concurrent with insulin or an insulin secretagogue (e.g., a sulfonylurea), blood glucose should be monitored, and a dose reduction in the insulin or the sulfonylurea may be necessary to avoid hypoglycemia. Rare cases of angioedema and anaphylaxis have been reported with semaglutide. Patients with diabetic retinopathy should be monitored for complications.
